I wish to take the Membership of the Consumer Rights Council / उपभोक्ता अधिकार परिषद् for which I am submitting the membership application fees of Rs. 2000/- (rupees two thousand only) along with my ID proof therefore kindly register me as a member of the council. I promise to be loyal towards the mission of the council and would follow all the rules and regulations which are given to me by council to fulfill its mission.

I undertake to abide by the Constitution, rules, and discipline of the Council. I do not believe in discrimination based on caste, sex, or religion. At the same time, for any misconduct, deviation of rules, or even for not following the disciplinary protocol of the organization, the National President may terminate my Membership. I confirm here that there is no criminal record or pending case against me at any police station in our country.

I do declare that the membership application fee and contribution (if any) are from my personal funds and are voluntary in nature.
